That you, whilst employed as a Band 6 Liaison Lead at The Sutherland Centre (“the Centre”) between 2008 and November 2009 did the following:

1. In March 2009, you breached professional boundaries in that you inappropriately gave Patient A your mobile telephone number and instructed him ‘to call you if he needed you’ or words to that effect;

2. In February or March 2009, you failed to maintain accurate records for Patient A in that you;

a. failed to create a care plan for Patient A and/or;
b. failed to record that a care plan was created for Patient A;

3. Between April and June 2009, you had an inappropriate relationship with Patient A in that you:

a. sent text messages to Patient A on his personal mobile telephone, in particular, you sent text messages to Patient A with words to the effect of ‘I miss you’, ‘xo’ or ‘love you’;
b. left voicemail messages on Patient A’s telephone saying words to the effect of ‘If you do not come and see me now then you will never see me again’ and ‘I will end the relationship and you will be alone because you will never see me again’;
c. on unknown dates between May and June 2009, took Patient A out to dinner alone, specifically,

i. you went out with Patient A to Frankie and Benny’s in Newtown;
ii. you went out with Patient A for a curry;

d. in May 2009:

i. took Patient A out to dinner with your friend;
ii. following the dinner mentioned in 3(d)(i) above, went to the Roebuck Inn at 21 Cheadle Road, Forsbrook, Stoke-on-Trent, ST11 9BX with Patient A, alone;

e. in May 2009, bought Patient A alcohol, specifically at:

i. the dinner referred to in paragraph 3(d)(i) above;
ii. the Roebuck Inn at 21 Cheadle Road, Forsbrook, Stoke-on-Trent, ST11 9BX (referred to in 3(d)(ii) above);
iii. the Holiday Inn Express next to Britannia Stadium in Stoke-On-Trent (“the Hotel”);
